The situation

What has happened

Liam Fitzgerald has applied for permit PTW-26-0424 to replace a leaking valve on the 150 millimetre steel main on the Princes Highway service road at Morwell. The isolation will cut supply to 68 customers including a bakery, a childcare centre and a small hospital kitchen. The Permit to Work and Isolation Policy requires manager approval for any outage over 50 customers, a proven isolation plan, control centre notification and a relight plan for every customer. The application is missing the customer count justification, the second isolation point is not identified, and no traffic management plan is attached. Sam Whitlock wants the permit ready for issue on Monday 21 September.

Evidence guide

The student must identify the missing second isolation point, the manager approval trigger and the traffic management gap. The corrected application must show isolation proven by reading rather than valve position. The relight plan must prioritise vulnerable customers and state how each relight is recorded. The approval request must include a contingency. Assess supervision judgement, not just form completion.
