Purpose. Crew leaders use this template to break a task into steps, identify the hazards at each step and record the controls before work starts.

1.When to use this template

Prepare a job safety analysis for every field task that is not fully covered by a current safe work method statement, and for any task where site conditions differ from the plan. Complete it on site with the crew and review it whenever the task changes.

2.Breaking the task into steps

List the steps in the order they will be performed, from arrival and site set-up to restoration and departure. Keep each step to one action so that hazards are not missed.

3.Identifying hazards

For each step consider gas, excavation, traffic, plant, manual handling, weather, underground services and the public. Use the site readings and the network drawing rather than assumptions.

  • Gas escape and ignition
  • Trench collapse and engulfment
  • Contact with other underground services
  • Traffic and mobile plant
  • Manual handling and noise

4.Selecting controls

Apply the hierarchy of control. Elimination and isolation come before administrative controls and personal protective equipment. Name the person responsible for each control and how it will be checked.

5.Sign on and review

Every crew member must read the analysis and sign on. The crew leader must review it at each break and after any change in conditions, and must file it with the work order.
